c2005. By Pat McPartland. This book was written specifically for special education teachers at the high school level, but parents will also find this manual very informative. All areas of the transition plan process are covered in this detailed resource, including interviewing students and parents, assessing needs, and transferring goals to an Individualized Education Program (IEP). Preparing students for life after high school is referred to in the Individuals with Disabilities Education Act (IDEA) as “transition services,” and this process is required for all students age 14 and older in the public school system. Guidelines are provided to help educators work with students and their families to create a workable plan that can be modified and re-assessed throughout high school. Flow charts are included for each grade level, and pre-assessment forms are also presented to help individualize goals and objectives for each student. In addition, detailed activity areas are listed for each of the seven transition areas: instruction; related services; community experiences; development of employment; post high school adult living; daily living skills; and functional assessment. There is also a sample case study portraying a fictional student, and all of the forms and assessments are completed and presented for each of the student’s four years of high school. The book also offers an appendix with reproducible blank copies of all sample forms, and it contains a CD-ROM with a printable PDF file of the book’s complete text. (Spiral bound; 134 pages.)
