Designed by teachers and therapists who are highly experienced in Structured Teaching methodology, this is a convenient and practical questionnaire set that provides questions for professionals to ask parents, caregivers, and students when developing educational programs for students with autism spectrum disorder (ASD) and other developmental disabilities. The areas covered in the parent/caregiver questionnaire include: expressive communication; listening; play and leisure; self-help skills; social development; routines; physical development; and medical issues. The last part of the questionnaire contains 12 questions to guide thinking about how home information can be used to determine goals and objectives, set up the physical environment, create visual structure, handle behavioral challenges, and transfer routines. In addition, using the questionnaire in a consistent manner throughout the year can aid in documenting progress across settings. According to the authors, it is important to encourage student participation by utilizing the included two-page student questionnaire, which can be used as a source of additional programming information. Teachers in a TEACCH-affiliated classroom can utilize the answers provided by parents, caregivers, and students to help create the students' educational and therapeutic programming.

This is Version 1 of the questionnaires in English, which has been designed for use with preschool and elementary learners. (Also available in Spanish.) The questionnaires are printed on brightly colored paper for easy access within a student's folder. Each package contains 10 parent and 10 student questionnaires, along with recommended guidelines for use. (Various pagings.)
